The history of handwashing in medicine offers a striking example of how evidence-based practices can face fierce resistance even when the stakes involve life and death. In the 1840s, Hungarian physician Ignaz Semmelweis observed that maternal mortality from puerperal fever dropped dramatically when doctors washed their hands with a chlorinated lime solution before examining patients, particularly after conducting autopsies. His data showed mortality rates falling from nearly 18 percent to around 1 percent in his obstetric clinic. Despite this compelling evidence, Semmelweis faced widespread ridicule and rejection from the medical establishment of his time.

The resistance stemmed from several factors rooted in the scientific understanding of the era. Germ theory had not yet been established, so doctors had no theoretical framework for why invisible contamination on their hands might cause disease. Many physicians found it professionally insulting to suggest that they themselves were transmitting fatal infections to patients. Semmelweis also struggled to communicate his findings persuasively and clashed with prominent colleagues, further alienating potential allies. He died in 1865, largely discredited, before his ideas gained mainstream acceptance.

It was only after Louis Pasteur and Robert Koch developed and popularized germ theory in the following decades that Semmelweis's findings gained retrospective validation and handwashing became standard medical practice. A common misconception is that his ideas were simply ahead of their time in a passive sense; in reality, they were actively and sometimes hostilely rejected by influential figures who prioritized professional pride and existing medical dogma over emerging statistical evidence.
